I am confirming that Motivation Academy agent is providing me with counseling/advice regarding my university application.When completed, this form my agent will submit to the university I am seeking enrolment at.

Upon receipt of my completed form, I am aware and authorize the university to remove my current agent ( if any) and change it in the student database to my proposed Agent Motivation Academy Based on my previous Privacy Declaration, (as applicable) all future correspondence will be sent to Motivation Academy. Any commission payment due in relation to my enrolment will be paid to Motivation Academy.

You must provide accurate and complete information when using our services. If you provide wrong, inaccurate or false information or documents your application may be rejected and your student finance may be affected.

1. Limitation of liability of incorrectly provided data.

1.1 Motivation Academy Limited is not responsible for validity and correctness of data provided by applicants. It is sole responsibility of applicants to ensure that all information provided are true and accurate.

Motivation Academy Limited will not accept liability for refused, disregarded, delayed, omitted or otherwise unsatisfactorily process application due to incorrect data or information provided by a student. 1.2 References to liability in this clause 1 include every kind of liability arising under or in connection with this agreement including but not limited to liability in contract, tort (including negligence), misrepresentation, restitution or otherwise.

1.3 Neither party may benefit from the limitations and exclusions set out in this clause in respect of any liability arising from its deliberate default.

1.4 Nothing in this agreement limits any liability which cannot legally be limited, including liability for:

(a) death or personal injury caused by negligence;

(b) fraud or fraudulent misrepresentation; and

(c) breach of the terms implied by section 12 of the Sale of Goods Act 1979 or section 2 of the Supply of Goods and Services Act 1982 (title and quiet possession).

2. Motivation Academy Limited do not provide financial services

2.1 We are not authorised and regulated to provide financial advice.

2.2 We do not provide financial, investment and other advice in relation to the student finance products available or otherwise.

2.3 We do not provide any recommendation or endorsement of products or services available.

2.4 Prior to making any financial related decision, you may wish to consult a financial adviser, accountant, lawyer or other professional adviser.

2.5 Motivation Academy Limited does not accept any liability as a consequence of making any student finance application. It remains your responsibility to check and understand terms of any financial commitment you undertake.

Before submitting this application, please ensure that you read and confirm each of the below statements, that you are clear about each of the policies surrounding your application, and ensure they you give explicit permission for your personal data to be shared with Motivation Academy Limited and named third parties.
